Shift Type: Monday-Friday, some weekends as needed; hours vary (primarily day shifts)

Roles & Responsibilities
- Provide high-quality nursing care to clinic patients, following established standards and your LPN scope of practice.
- Serve as the primary liaison between patients, providers, and clinic staff to ensure smooth communication and continuity of care.
- Take and interpret vital signs and perform bedside assessments to support clinical decision-making.
- Triage patient calls, determine urgency, and coordinate next steps with providers and the care team.
- Process medication refills and complete prior authorizations accurately and promptly.
- Support and direct CNAs and Schedulers in daily workflows to keep the clinic efficient and patient-centered.
- Collaborate with RNs, Providers, Coordinators, and Clinical Managers to meet patient needs and improve care delivery.
Qualifications
- Graduate of an accredited nursing program (LPN)
- Current State LPN license required
- Basic Life Support (BLS) certification required
- Comfortable triaging calls and managing vital signs and clinical assessments
- Experience with medication refills, prior authorizations, and care coordination preferred
- Strong communication skills and ability to provide direction to CNAs and Schedulers
Education
- Graduate of a Nursing Program
Knowledge & Skills
- Clinical nursing fundamentals and outpatient/urgent care workflows
- Triage protocols and effective patient assessment
- Taking and interpreting vital signs
- Medication refill processes and prior authorization workflows
- Care coordination and team-based communication
- Supervisory direction for support staff (CNAs, Schedulers)
