Roles & Responsibilities
- Provide compassionate physical therapy services directly in patients' homes across Yolo County
- Assess and evaluate each patient's therapeutic needs while developing personalized care plans
- Guide patients and families through rehabilitation exercises and therapeutic appliance use
- Monitor progress and communicate changes to physicians while maintaining detailed clinical records
- Train and supervise home health aides participating in patient rehabilitation programs
- Connect patients with additional home health resources when needed for comprehensive care
Qualifications
- Minimum 2 years experience as a Physical Therapist in acute, home care, or rehab settings
- Current BLS certification required
- Strong oral and written communication skills with excellent organization abilities
- Valid driver's license and reliable transportation for home visits throughout the service area

- Physical therapy assessment and treatment planning
- Home environment evaluation and safety assessment
- Patient and family education techniques
- Prosthetic device training and rehabilitation supervision
- Medicare and Medi-Cal compliance knowledge
- Clinical documentation and progress reporting
