Shift Type: Monday-Friday, 8:00 AM - 5:00 PM
Hours per Week: 40
Candidate Pay Rate: $70 per hour

Roles & Responsibilities
- Provide compassionate physical therapy services to patients in their homes throughout Tehama County
- Conduct Start of Care visits especially for orthopedic patients when nursing is not ordered
- Develop personalized treatment plans for patients recovering from joint replacements and managing chronic conditions like CHF and COPD
- Complete approximately 5 meaningful patient visits each day using your clinical expertise
- Document care efficiently using the HomeCare HomeBase EMR system on your provided tablet
- Collaborate with the intake RN and office manager to coordinate exceptional patient care
Qualifications
- Current Physical Therapy license in California
- BLS certification required
- Home Health experience preferred but will consider candidates with less than 2 years experience who have Home Health background
- Strong assessment and clinical skills for diverse patient populations
Knowledge & Skills
- Physical Therapy expertise in home health settings
- Experience with orthopedic and geriatric patient populations
- Comfortable working independently in home environments
- Proficiency with EMR documentation systems
- Excellent communication and patient education abilities
