Contract Type: Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM with every 3rd weekend rotation

Hours per Week: 40

Candidate Pay Rate: $64 per hour

Roles & Responsibilities

Deliver personalized physical therapy services in patients' homes across your assigned geographic region
Evaluate and treat patients with orthopedic conditions, strokes, joint replacements, and spinal injuries
Complete OASIS documentation accurately and on time as required by government regulations
Collaborate with PTAs, OTAs, and rehab aides to coordinate comprehensive care plans
Build trusting relationships with patients and families while helping them achieve their mobility goals

Qualifications

Active Physical Therapist license in California
Current BLS certification required
Minimum one year of physical therapy experience
Experience with OASIS documentation required
Home health experience preferred but not required
Cerner EMR experience preferred

Knowledge & Skills

Strong assessment and treatment planning abilities
Excellent communication and patient education skills
Comfort working independently in home settings
Proficiency with OASIS and Cerner EMR documentation
Compassionate approach to geriatric and post-acute care
