Roles & Responsibilities

• Perform thorough patient assessments and document findings accurately
• Administer medications and treatments as prescribed
• Communicate effectively with physicians and nurse practitioners regarding patient status
• Provide compassionate ADL assistance and support for daily living
• Manage specialized care including gastrostomy tubes, urinary catheters, wound vacs, CPAP, and tracheostomy
• Coordinate admissions, discharges, and transitions of care with the healthcare team

Qualifications

• Active LPN license in Hawaii or compact state (upon acceptance)
• Minimum 1 year of LPN experience in a long-term care or similar setting
• Current BLS/BCLS certification required
• Proof of flu vaccination prior to start
• Flexibility to work varied shifts including weekends and holidays

Knowledge & Skills

• Proficient in patient assessments and clinical documentation
• Skilled in medication administration and treatment protocols
• Experienced with ADL assistance and patient mobility
• Knowledge of gastrostomy tube, urinary catheter, and wound care management
• Competent in nebulized treatments, CPAP, and tracheostomy care
• Strong communication and interpersonal skills for team collaboration
