Roles & Responsibilities

• Provide compassionate, resident-centered care in a long-term care setting
• Administer medications and treatments accurately and safely
• Monitor and document vital signs, symptoms, and changes in condition
• Collaborate with nurses, physicians, and the interdisciplinary care team
• Educate residents and families on care plans and health management
• Maintain a safe, clean, and supportive environment for all residents

Qualifications

• Current, active LPN or RN license in New Hampshire or a compact state
• BLS/CPR certification (American Heart Association or equivalent)
• Experience in long-term care, skilled nursing, or geriatrics preferred
• Must have completed COVID-19 vaccine or an approved medical/religious exemption
• Ability to pass background checks and provide proper identification
• Compassionate, reliable, and ready to make a difference

Knowledge & Skills

• Strong clinical assessment and critical thinking skills
• Proficient in medication administration and accurate documentation
• Excellent communication and interpersonal skills
• Comfortable using electronic health records (EHR) systems
• Ability to adapt quickly to new environments and workflows
• Team collaboration and a focus on patient advocacy
