Roles & Responsibilities

• Provide compassionate personal care and assistance with activities of daily living to residents
• Take and record vital signs accurately and report any changes to the nursing team
• Assist with safe transfers, ambulation, and mobility support while promoting resident independence
• Document resident care and observations thoroughly and communicate clearly with the care team
• Maintain a clean, safe environment and follow infection control and safety protocols
• Build meaningful relationships with residents and their families to support quality of life

Qualifications

• Current LNA / Licensed Nursing Assistant certification (VT or NH as applicable)
• Completion of an approved nurse assistant program
• Comfortable working in long-term care or skilled nursing environments
• Compassionate demeanor, strong communication skills, and a team-player attitude
• Ability to pass background checks and meet facility immunization/health requirements

Education

• High school diploma or GED and completion of an approved LNA/nurse aide program

Knowledge & Skills

• Assisting with ADLs (bathing, dressing, toileting, feeding)
• Taking and accurately recording vital signs
• Basic infection control and safety procedures
• Resident-centered communication and documentation
• Safe transfer and mobility assistance techniques
• Team collaboration and following care plans
