Home Health RN
Location: St Johnsbury, VT

Contract Type: Travel Contract
Shift Type: Monday-Friday, 7:00 AM - 3:00 PM, possible weekend shifts
Hours per Week: 36
Candidate Pay Rate: $70 per hour

Roles & Responsibilities
- Provide compassionate nursing care to clients of all ages in their homes
- Conduct thorough admissions, evaluations, and develop personalized care plans
- Collaborate with a dedicated team of homecare professionals to deliver coordinated care
- Support clients and families throughout Caledonia and southern Essex counties
- Strive to achieve the highest level of patient-centered outcomes every day
Qualifications
- Current Vermont RN license required
- At least one year of prior clinical experience
- ACLS, BLS/BCLS, PALS, NRP, CPI, ENPC, RNFA, TNCC, AWHONN, STABLE, ASLS certifications preferred
- Strong assessment and care-planning skills
- Excellent communication and teamwork abilities

- Home health nursing expertise
- Patient assessment and care coordination
- Comfort working independently in community settings
- Ability to build trusting relationships with clients and families
