Roles & Responsibilities

• Manage and coordinate patient care plans across the continuum
• Collaborate with interdisciplinary teams to ensure optimal outcomes
• Conduct assessments, discharge planning, and resource coordination
• Advocate for patients and families to navigate healthcare services
• Utilize evidence-based practices to improve care quality and efficiency

Qualifications

• Must have acute care case management experience (non-negotiable)
• Preferred experience with Meditech, Midas, and HCA systems
• Current RN license in Florida or compact state
• Minimum 2 years of recent case management experience

Knowledge & Skills

• Strong clinical assessment and critical thinking skills
• Excellent communication and interdisciplinary collaboration abilities
• Proficient in electronic medical records and case management software
• Knowledge of discharge planning, utilization review, and care coordination
