Roles & Responsibilities

• Conduct clinical screening, assessment, diagnosis, and treatment planning for patients from birth through elder.
• Deliver brief therapy and continuing care interventions to individuals and families.
• Lead and coordinate inpatient discharge planning, including transitions to ALF or transitional placements.
• Facilitate or co-facilitate patient care conferences and collaborate with multidisciplinary teams.
• Provide care coordination, client advocacy, and assistance with complex legal matters such as Power of Attorney when relevant.
• Assess chemical dependency and mental health needs and make appropriate referrals.
• Maintain accurate, timely, and compliant documentation to meet state, federal, accreditation, and hospital billing standards.
• Participate in SEARHC Quality Management trainings and integrate improvements into client services.
• Assist with provider enrollment, billing duties, and other administrative requirements as assigned.

Qualifications

• Master's degree in Social Work, Psychology, Counseling, or related discipline from an accredited institution
• Licensed or in the process of becoming licensed as an LCSW, LPC, or LMFT
• Valid State of Alaska Driver's License or ability to obtain one within 6 months of hire
• Must be 21 years of age
• Clinical competency required within 3 months of hire and ongoing every three years
• Ability to perform case management, crisis intervention, and culturally competent care for Tribal communities

Education

• Master's degree in Social Work, Psychology, Counseling, or related discipline (required)
• Clinical Competency Assessment - SEARHC (required)

Knowledge & Skills

• Discharge planning and care coordination across the continuum
• Clinical assessment, documentation, and treatment planning
• Crisis intervention and interviewing techniques
• Understanding of accreditation, state/federal regulations, and hospital billing requirements
• Experience with chemical dependency assessment and referral processes
• Strength in multidisciplinary collaboration and client advocacy
• Cultural/Tribal health competency and sensitivity
