Roles & Responsibilities

• Provide diabetes education and case management for patients with diabetes, prediabetes, and gestational diabetes to improve outcomes and self-management
• Maintain and manage diabetes registries to ensure patients receive recommended annual exams, labs, immunizations, and education
• Coordinate primary and secondary prevention efforts with the SEARHC Consortium Diabetes team
• Lead and coordinate completion of the annual IHS Diabetes Audit and use RPMS tools and audit data to drive quality improvements
• Deliver training and education on IHS Standards of Care for medical providers, dietitians, nurses, and related staff
• Support and model SEARHC's Seven Standards of Excellence in daily practice
• Respond professionally to inquiries from staff, providers, patients, and visitors and maintain effective communication across departments

Qualifications

• Active, valid Registered Nurse (RN) license in the state of Alaska
• Certification as a Diabetes Educator OR eligibility to apply for certification within 2 years
• Clinical competency to be completed within 3 months of hire and every three years thereafter
• Preferred: 3 years general nursing or dietetic experience
• Preferred: 1 year experience in diabetic care and management
• Ability to perform physical demands of role, including lifting/moving up to 50 lbs

Education

• Registered Nurse (RN) licensure — State of Alaska
• Certification as a Diabetes Educator (or eligibility within 2 years); Clinical Competency assessment per SEARHC

Knowledge & Skills

• Expertise in diabetes case management practices and patient education
• Familiarity with IHS Standards of Care for Adults with Type 2 Diabetes
• Experience using RPMS and diabetes registries
• Strong teaching, organization, and communication skills
• Cultural competence and knowledge of Alaska Native customs and geographic considerations
• Comfort with computer systems and electronic health records
• Ability to lead group programs and engage patients in self-management
