Roles & Responsibilities

• Provide skilled physical therapy evaluation and treatment to residents in a skilled nursing facility
• Develop and implement individualized, goal-driven treatment plans
• Assess, document, and report patient progress in a timely and compliant manner
• Collaborate with the interdisciplinary team to coordinate care and optimize outcomes
• Meet facility productivity expectations (90% productivity requirement as stated)
• Adhere to Continuum Rehabilitation Group policies and facility procedures
• Maintain professional appearance (black scrubs as dress code) and be present in the building to receive guaranteed hours
• Communicate effectively with families, nursing staff, and leadership to support patient care

Qualifications

• Licensed Physical Therapist (PT) in New Mexico or eligible for licensure
• DPT or equivalent physical therapy degree (DPT listed among desired credentials)
• 1-3 years of experience (as indicated in the job posting), experience in SNF/geriatric care preferred
• Strong clinical documentation skills and ability to meet productivity targets
• Comfortable working as part of a close-knit rehab team and following CRG policies

Education

• Doctor of Physical Therapy (DPT) preferred or equivalent PT degree

Knowledge & Skills

• Skilled Nursing Facility (SNF) care and geriatric rehabilitation expertise
• Patient assessment, mobility and gait training, and therapeutic exercise planning
• Experience with common rehab equipment and safe patient handling
• Proficiency in clinical documentation and productivity tracking
• Excellent communication, teamwork, and time-management skills
• Ability to follow facility policies and clinical best practices
