Roles & Responsibilities

Provide compassionate, patient-centered physical therapy services to assigned patients
Perform thorough evaluations and document functional status and progress
Develop and implement individualized treatment plans that promote mobility and independence
Collaborate with interdisciplinary staff to coordinate holistic care
Document interventions and outcomes accurately in the medical record
Participate in orientation and site-specific training to ensure consistent, high-quality care

Qualifications

Current, active California physical therapy license (required)
Basic Life Support (BLS/CPR) certification, or willingness to obtain
Comfort working in a team-oriented healthcare setting; orientation provided
Strong communication skills and patient education ability
Ability to pass credentialing and background checks as required by the facility

Education

Doctor of Physical Therapy (DPT) or equivalent degree in physical therapy

Knowledge & Skills

Skilled in therapeutic exercise, functional mobility training and patient education
Familiarity with clinic documentation and EMR workflows
Ability to assess and modify treatment plans based on patient progress
Strong interpersonal skills for working with patients, families and care teams
Time management and organizational skills to manage a part-time caseload
