Roles & Responsibilities

Treat patients with movement dysfunction or pain to improve function, reduce fall risk, and support recovery from surgery.
Perform thorough patient evaluations and create therapeutic treatment plans focused on improving functional mobility and independence.
Document all patient care activities clearly and promptly, including evaluations, progress notes, daily notes, discharges, and insurance communications.
Lead and support discharge planning to ensure safe transitions and optimal patient outcomes.
Communicate professionally with other providers, insurers, patients, rehab staff, and supervisors to coordinate care and manage authorizations.
Assist with maintaining a safe, clean, and organized treatment area by helping with treatment room cleaning, laundry, and related tasks.
Perform other duties as requested to support the rehab team and clinic operations.

Qualifications

Valid Physical Therapist licensure in Wisconsin or eligibility to obtain state licensure.
Graduate of an accredited Physical Therapy program (DPT or equivalent preferred).
Proven ability to perform patient assessments, create treatment plans, and document care thoroughly.
Strong communication skills for interacting with patients, providers, and insurance representatives.
Comfortable assisting with light clinic upkeep (treatment room cleaning, laundry) as part of a team-oriented environment.

Education

Doctor of Physical Therapy (DPT) or degree in Physical Therapy (preferred)

Knowledge & Skills

Patient evaluation and assessment
Therapeutic interventions and treatment planning
Progress notes and discharge documentation
Pain management strategies
Fall risk reduction techniques
Functional mobility improvement
Experience with insurance approvals/denials communication
Clear, professional communication with providers and payers
