Roles & Responsibilities

Perform thorough patient assessments in the home environment and identify functional needs
Develop and implement individualized treatment plans that promote independence
Deliver skilled therapeutic interventions focused on mobility, strength and safety
Document visits clearly and promptly in the EMR and VMS platform
Collaborate with interdisciplinary team members and communicate progress with families
Educate patients and caregivers on home safety, assistive devices and fall prevention
Monitor outcomes and adjust care plans to ensure meaningful progress

Qualifications

Active Physical Therapist (PT) license or eligibility for state licensure
Clinical experience in physical therapy; home health experience is a strong plus
BLS/CPR certification or ability to obtain
Comfortable working independently in patient homes with reliable transportation
Strong communication skills and professional bedside manner

Education

Doctor of Physical Therapy (DPT) preferred; Bachelor of Science in Physical Therapy or related degree accepted

Knowledge & Skills

Skilled in patient assessment, gait training, balance and mobility interventions
Experience with OASIS/EMR documentation and familiarity with VMS platforms
Knowledge of home safety evaluations and ADL training
Excellent time management and caseload prioritization abilities
Ability to educate patients and caregivers in an approachable, motivational way
