Roles & Responsibilities

• Provide skilled physical therapy evaluations and interventions in patients’ homes
• Develop individualized treatment plans focused on functional independence
• Perform mobility, balance and safety assessments and implement fall-prevention strategies
• Coordinate care with nurses, aides and the interdisciplinary home health team
• Document clinical care and progress thoroughly in HCHB
• Educate patients and families on home exercise programs, safe transfers and equipment use
• Manage a daily caseload of ~5–6 patients while maintaining high-quality documentation
• Communicate changes in patient status promptly to the care team to optimize outcomes

Qualifications

• Active Physical Therapist license in North Carolina (or compact/reciprocity as applicable)
• Minimum 2 years clinical experience (home health experience strongly preferred)
• Demonstrated home health clinical skills and comfort with hospice/home care populations
• Familiarity with HCHB documentation system (Home Care Home Base)
• Valid driver’s license and reliable vehicle for patient visits
• Strong interpersonal and patient-education skills; ability to work independently

Education

• Doctor of Physical Therapy (DPT) or equivalent degree and active PT licensure

Knowledge & Skills

• Skilled in home assessment, mobility and balance interventions
• Proficient with HCHB or similar home health documentation systems
• Excellent time management and travel-route planning for community visits
• Ability to collaborate with interdisciplinary teams and communicate clinically
• Knowledge of home safety, adaptive equipment recommendations and caregiver training
• Comfort working with hospice and medically complex home health patients
