Shift Type: Monday-Friday, 8:00 AM - 4:00 PM or 9:00 AM - 5:00 PM; weekends as needed

Candidate Pay Rate: $56 per hour

Roles & Responsibilities

• Provide exceptional physical therapy care to patients in the comfort of their homes
• Develop and implement individualized treatment plans to improve mobility and independence
• Collaborate with patients, families, and healthcare teams to achieve optimal outcomes
• Educate patients and caregivers on therapeutic exercises and fall prevention
• Maintain accurate and timely documentation of patient progress

Qualifications

• Active Physical Therapist license in the state of Iowa
• BLS certification
• Experience in home health or rehabilitation settings
• Ability to work independently and adapt to diverse home environments
• Experience with Epic EMR preferred

Knowledge & Skills

• Strong knowledge of physical therapy techniques and modalities
• Understanding of home health care regulations and best practices
• Excellent communication and interpersonal skills
• Proficient in patient assessment and care planning
• Familiarity with electronic medical records (Epic preferred)
