Roles & Responsibilities

• Assess patients and observe treatment effects to recommend changes to physicians and care teams
• Perform appropriate physical therapy assessments for neuromuscular, skeletal, integumentary, pulmonary, and cardiovascular disorders
• Develop and implement individualized treatment plans to improve and restore function
• Select and apply therapeutic procedures to maintain, improve, and restore patient mobility and independence
• Document patient progress and communicate clinical findings with physicians and interdisciplinary staff
• Provide compassionate, patient-centered care and educate patients and families on home programs and prevention strategies

Qualifications

• Current Colorado Physical Therapy license required
• Basic Life Support (American Heart Association) certification required
• Willingness to relocate to a rural area and join the San Luis Valley community
• Ability to provide care across a range of conditions including neuromuscular, skeletal, integumentary, pulmonary, and cardiovascular disorders
• Available for full-time day-shift work

Knowledge & Skills

• Patient assessment and treatment planning
• Therapeutic interventions and rehabilitation techniques
• Clinical documentation and progress tracking
• Experience with neuromuscular, skeletal, integumentary, pulmonary, and cardiovascular conditions
• Strong communication and interdisciplinary collaboration skills
• Ability to adapt treatment based on observed patient response
