Roles & Responsibilities

Provide compassionate, hands-on nursing care to residents/patients with dignity and respect
Administer medications and treatments accurately and document per policy
Monitor and record vital signs, changes in condition, and communicate concerns promptly
Collaborate with nurses, therapists, and the care team to deliver coordinated care
Educate patients and families on care plans, medications, and safe practices
Maintain accurate clinical records and support quality and compliance efforts
Follow infection control procedures and facility safety guidelines at all times

Qualifications

Current, active LPN license (eligible to practice in Pennsylvania required)
CPR/BLS certification (or willingness to obtain upon hire)
Comfortable with hands-on patient care and basic nursing procedures
Strong communication skills and the ability to work well within a care team
Reliable, professional, and committed to maintaining patient confidentiality

Education

Graduate of an accredited Practical Nursing (LPN) program
High school diploma or equivalent

Knowledge & Skills

Skilled in medication administration and documentation
Solid patient assessment and monitoring skills
Familiarity with care planning and basic nursing interventions
Ability to chart clearly and use electronic health records or documentation systems
Good time management, prioritization, and teamwork abilities
