Roles & Responsibilities
- Provide compassionate direct patient care following care plans and facility policies
- Administer medications and treatments as directed within LPN scope
- Monitor patient condition and promptly report changes to the RN or provider
- Document care, observations, and patient responses accurately and timely
- Collaborate with the care team to ensure safe, high-quality patient outcomes
- Assist residents with activities of daily living and promote a comfortable environment
Qualifications
- Active LPN license in Pennsylvania (or applicable compact state credentials)
- Graduation from an accredited practical nursing program
- Current CPR/BLS certification preferred
- Ability to pass background checks and meet facility onboarding requirements
- Strong communication skills, compassion, and ability to work well on a team
Education
- High school diploma or GED; completion of an accredited practical nursing program preferred
Knowledge & Skills
- Medication administration and basic nursing procedures
- Patient assessment and vital signs monitoring
- Clinical documentation and charting
- Infection control and safety practices
- Effective interpersonal communication and resident-centered care
