Roles & Responsibilities

Provide compassionate, hands-on nursing care to residents, focusing on dignity and safety
Administer medications and treatments accurately following physician orders and facility policy
Monitor and document vital signs, patient status changes, and care provided in the medical record
Collaborate with the care team to develop and implement individualized care plans
Assist with activities of daily living while promoting independence and comfort
Communicate clearly with residents, families, and interdisciplinary team members to ensure quality care

Qualifications

Current, active LPN license in Pennsylvania
Basic Life Support (BLS/CPR) certification or willingness to obtain upon hire
Comfortable administering medications and providing direct patient care
Strong communication and teamwork skills, with a patient-centered attitude
Previous experience in long-term care or skilled nursing preferred

Education

Completion of an accredited Practical Nursing program (LPN diploma or certificate)

Knowledge & Skills

Medication administration and basic IV/SC support knowledge
Vital sign assessment and clinical observation skills
Familiarity with electronic medical records and documentation best practices
Wound care basics and pressure injury prevention
Ability to prioritize tasks in a fast-paced care environment
