Roles & Responsibilities
- Provide compassionate, hands-on nursing care to residents and patients
- Administer medications and treatments accurately and on schedule
- Document patient status, interventions and outcomes in the electronic health record
- Monitor patient condition and promptly report changes to the nursing team and providers
- Collaborate with nurses, therapists and support staff to deliver coordinated care
- Assist with admissions, discharges and safe patient transfers
- Educate patients and families about care plans, medications and follow-up
Qualifications
- Current Licensed Practical Nurse (LPN) license in Pennsylvania required
- Current BLS/CPR certification required
- Completion of an accredited practical/vocational nursing program
- Background check, immunizations and TB screening as required by the facility
- Strong communication skills and the ability to work well in a team
- Previous LPN experience in long term care or similar settings preferred
Education
- Graduate of an accredited Practical Nursing (LPN) or Vocational Nursing program
Knowledge & Skills
- Medication administration and basic clinical procedures
- Patient assessment and vital signs monitoring
- Basic wound care and infection control practices
- Accurate and timely electronic charting
- Time management and prioritization in a fast-paced setting
- Compassionate patient- and family-centered care
