Roles & Responsibilities

Provide direct nursing care to clinic patients following established standards and within scope of LPN licensure.
Serve as the primary liaison between patients, providers and clinic staff to ensure coordinated, patient-centered care.
Take and record vital signs, perform patient assessments and triage patient calls efficiently and compassionately.
Manage medication refills and complete prior authorization paperwork to keep patient treatments on track.
Give direction and guidance to CNAs and coordinate with Schedulers to streamline clinic flow.
Collaborate with Providers, RNs, Coordinators and Clinical Managers to support team goals and excellent patient outcomes.

Qualifications

Graduate of an accredited nursing program (LPN diploma/certificate).
Current State LPN license (must be active and in good standing).
BLS (Basic Life Support) certification — as specified from an accredited college/technology or nursing program.
Ability to triage, take vital signs, manage medication refills and complete prior authorizations.
Comfort supervising CNAs and coordinating with schedulers and clinical staff.
Willingness to meet ongoing continuing education requirements as dictated by state/federal licensure.

Education

Graduate of a Nursing Program (LPN)

Knowledge & Skills

Patient assessment and vital signs monitoring
Triage and telephone triage skills
Medication management and refill processing
Prior authorization workflows and documentation
Basic Life Support (BLS) competency
Strong communication and patient liaison skills
Team collaboration with Providers, RNs, Coordinators and Managers
Experience working in outpatient or walk-in clinic environments
