Roles & Responsibilities
- Provide nursing care for clinic patients following established standards and scope of practice
- Serve as the primary liaison between patients, staff, and providers to ensure coordinated care
- Perform patient assessments and take vital signs accurately and efficiently
- Triage patient calls, determine appropriate next steps, and escalate to providers as needed
- Manage medication tasks including prescription refills and completing prior authorizations
- Give direction to CNAs and work collaboratively with schedulers to support clinic flow
- Collaborate with interdisciplinary team members to meet patient needs and improve outcomes
Qualifications
- Graduate of a Nursing Program
- Current State LPN license
- BLS (Basic Life Support) certification
- Ability to work within the scope of licensure and follow state/federal continuing education requirements
- Experience in triage, vital signs, medication management, and prior authorization processes preferred
Education
- Graduate of a Nursing Program (practical nursing/LPN education)
