Roles & Responsibilities
- Provide compassionate direct patient care and support activities of daily living
- Administer medications and treatments according to care plans and facility policies
- Monitor and document patient status, changes, and responses to interventions
- Collaborate with nurses, therapists, and care team members to ensure coordinated care
- Educate patients and families about care plans, medications, and safety
- Maintain accurate, timely charting in the facility electronic or paper records
- Adhere to infection control standards and facility safety protocols
Qualifications
- Current Licensed Practical Nurse (LPN) license — Pennsylvania licensure expected for this facility
- Basic Life Support (BLS) certification preferred or obtained upon hire
- Comfortable with medication administration, basic assessments, and documentation
- Ability to pass background checks and meet facility immunization requirements
- Strong communication skills and a collaborative, patient-first attitude
Education
- LPN diploma, certificate, or equivalent practical nursing program completion
Knowledge & Skills
- Skilled in patient assessments and basic nursing interventions
- Medication administration and documentation accuracy
- Familiarity with electronic medical records or skilled at accurate charting
- Infection control practices and safe care techniques
- Effective communication with patients, families, and the interdisciplinary team
- Time management and prioritization in a busy clinical setting
