Roles & Responsibilities
- Provide compassionate direct patient care consistent with LPN scope of practice
- Administer medications and treatments accurately and document observations
- Monitor and report changes in resident condition to the care team promptly
- Collaborate with RNs, CNAs, and interdisciplinary staff to create a safe care environment
- Maintain accurate, timely charting in electronic or paper medical records
- Support residents' activities of daily living with respect, dignity, and professionalism
Qualifications
- Current LPN licensure (Pennsylvania or compact/state-appropriate) — required
- Graduation from an accredited practical nursing program
- Basic Life Support (BLS/CPR) certification preferred or required per facility policy
- Previous experience in long-term care or skilled nursing is a plus
- Strong communication skills and the ability to work well in a team-focused environment
Education
- Graduate of an accredited LPN/Vocational Nursing program
- High school diploma or equivalent
Knowledge & Skills
- Fundamentals of nursing care and medication administration
- Charting and documentation best practices
- Patient assessment and recognition of clinical changes
- Infection control and safety protocols
- Effective interpersonal skills with residents, families, and staff
