Roles & Responsibilities
- Provide compassionate direct patient care, including administering medications and treatments as ordered
- Monitor and record vital signs, promptly reporting any changes to the healthcare team
- Assist with daily living activities and ensure patient comfort and safety
- Document patient care accurately and timely in the electronic medical record
- Collaborate with registered nurses and other healthcare professionals to deliver high-quality care
Qualifications
- Current and active Licensed Practical Nurse (LPN) license in Pennsylvania
- BLS/CPR certification required (or ability to obtain prior to start)
- Previous LPN experience in a long-term care or skilled nursing setting is preferred but not required
- Strong clinical assessment and documentation skills
- A compassionate, patient-centered approach to care
Knowledge & Skills
- Knowledge of medication administration and clinical nursing procedures
- Ability to assess patient conditions and respond appropriately
- Strong communication and interpersonal skills for collaborating with patients, families, and team members
- Proficiency with electronic health records (EHR) and documentation
- Ability to adapt quickly to changing patient needs and unit demands
