Roles & Responsibilities

• Provide compassionate direct patient care, including administering medications and treatments as ordered
• Monitor and record vital signs, promptly reporting any changes to the healthcare team
• Assist with daily living activities and ensure patient comfort and safety
• Document patient care accurately and timely in the electronic medical record
• Collaborate with registered nurses and other healthcare professionals to deliver high-quality care

Qualifications

• Current and active Licensed Practical Nurse (LPN) license in Pennsylvania
• BLS/CPR certification required (or ability to obtain prior to start)
• Previous LPN experience in a long-term care or skilled nursing setting is preferred but not required
• Strong clinical assessment and documentation skills
• A compassionate, patient-centered approach to care

Knowledge & Skills

• Knowledge of medication administration and clinical nursing procedures
• Ability to assess patient conditions and respond appropriately
• Strong communication and interpersonal skills for collaborating with patients, families, and team members
• Proficiency with electronic health records (EHR) and documentation
• Ability to adapt quickly to changing patient needs and unit demands
