Roles & Responsibilities

• Provide compassionate, resident-centered care in a skilled nursing facility
• Administer medications and treatments as ordered by physicians
• Monitor vital signs and observe changes in resident condition
• Collaborate with RNs and the healthcare team to implement care plans
• Document care accurately and timely in the medical record
• Communicate openly with residents and their families
• Maintain a safe, clean, and comforting environment

Qualifications

• Active Licensed Practical Nurse (LPN) license in Pennsylvania
• Current BLS/CPR certification
• Skilled nursing or long-term care experience preferred
• Strong clinical assessment and critical thinking skills
• Excellent communication and teamwork abilities
• Ability to work night shifts and adapt to facility needs
• Must pass background check and health screening

• Medication administration and documentation
• Resident assessment and care planning
• Wound care and infection control practices
• Electronic health records (EHR) documentation
• Compassionate patient education and support
• Effective collaboration with interdisciplinary teams
