Roles & Responsibilities
- Provide compassionate direct patient care and support residents with daily living activities
- Administer medications and document care accurately according to policy
- Monitor and record vital signs, reporting any changes to the care team promptly
- Collaborate with RNs, CNAs, and interdisciplinary staff to deliver coordinated care
- Maintain accurate charts and patient records, ensuring regulatory compliance
- Support a safe, clean, and respectful environment for residents and families
Qualifications
- Active LPN license in Pennsylvania (must have or be eligible)
- Current BLS/CPR certification or willingness to obtain
- Comfort working 8-hour shifts and performing hands-on patient care
- Strong communication skills and a patient-focused attitude
Education
- LPN diploma or practical nursing certificate from an accredited program
Knowledge & Skills
- Medication administration and basic clinical assessments
- Infection control practices and safety procedures
- Patient documentation and basic electronic charting familiarity
- Ability to prioritize, multitask, and work collaboratively
