Roles & Responsibilities
- Provide compassionate direct patient care, ensuring dignity and comfort
- Administer medications and treatments accurately and safely
- Monitor patient conditions and communicate changes to the care team
- Document care clearly and timely in the patient chart
- Assist with activities of daily living and mobility as needed
- Collaborate with nurses and interdisciplinary staff to deliver excellent care
Qualifications
- Active LPN license in Pennsylvania (or compact/reciprocal where applicable)
- Current BLS/CPR certification or willingness to obtain promptly
- Demonstrated clinical skills in medication administration, wound care, and basic assessments
- Strong communication and teamwork mindset—patients and coworkers appreciate it
- Ability to pass background and health screenings per facility policy
Education
- Completion of an accredited practical nursing program (LPN/LVN diploma or certificate)
Knowledge & Skills
- Solid knowledge of basic nursing principles and infection control
- Proficiency with medication administration and documentation
- Comfortable with electronic charting or willingness to learn
- Good assessment skills and ability to escalate concerns appropriately
- Strong interpersonal skills and a person-centered approach to care
