Roles & Responsibilities
- Provide compassionate direct care to residents as a Licensed Practical Nurse
- Administer medications and treatments accurately and timely
- Monitor patient condition and report changes to the care team
- Document assessments, interventions, and patient responses in the chart
- Collaborate with nurses, therapists, and support staff to deliver coordinated care
- Support admission, discharge, and care planning activities
- Maintain a safe, clean, and respectful environment for residents
Qualifications
- Valid LPN license in Pennsylvania (required)
- Current BLS/CPR certification or willingness to obtain
- Up-to-date immunizations and TB screening per facility policy
- Strong communication and interpersonal skills
- Comfortable with electronic medical records and standard charting
- Previous experience in long-term care or skilled nursing helpful but not required
Education
- LPN diploma, certificate, or vocational nursing program completion
Knowledge & Skills
- Medication administration and basic clinical procedures
- Wound care fundamentals and monitoring for changes
- Vital signs assessment and documentation
- Time management and prioritization in a busy clinical setting
- Teamwork, delegation, and clear communication with interdisciplinary staff
- Basic computer skills for charting and electronic documentation
