Roles & Responsibilities
- Provide compassionate, resident-centered nursing care as an LPN
- Administer medications and treatments accurately and safely
- Monitor resident condition and communicate changes to the care team
- Document care activities clearly and timely in resident records
- Collaborate with RNs, CNAs, and interdisciplinary staff to deliver excellent care
- Support resident dignity and independence through personalized care plans
Qualifications
- Current Licensed Practical Nurse (LPN) license in Pennsylvania (required)
- Basic Life Support (BLS) certification or willingness to obtain
- Demonstrated ability to work effectively in a team-focused care setting
- Strong communication and documentation skills
- Reliable, compassionate, and resident-centered approach to care
Education
- LPN diploma or certificate from an accredited program
- High school diploma or GED
Knowledge & Skills
- Knowledge of basic nursing practices and medication administration
- Familiarity with long-term care or skilled nursing environments
- Ability to accurately chart and follow care plans
- Strong interpersonal skills and ability to engage residents respectfully
- Basic clinical assessment and infection control practices
