Roles & Responsibilities

• Provide compassionate, hands-on care to residents in a long-term care setting
• Administer medications and treatments as ordered
• Monitor vital signs and report changes in patient condition
• Collaborate with nurses, physicians, and care team members
• Document patient care accurately in the medical record
• Support patients and families with education and encouragement

Qualifications

• Active Licensed Practical Nurse (LPN) license in New Mexico
• Current BLS certification (American Heart Association)
• COVID-19 vaccination or documented medical/religious exemption
• Government-issued identification and ability to wear a photo badge
• Previous long-term care or rehabilitation experience preferred, not required

Knowledge & Skills

• Strong patient assessment and clinical reasoning skills
• Excellent communication and interpersonal abilities
• Proficient in medication administration and documentation
• Comfortable using electronic health records (EHR)
• Team player with a positive, adaptable attitude
