Shift Type: Evening shift, 3:00 PM - 11:30 PM (30-minute meal break); alternating weekly schedule: Week 1 - Sunday 7:00 AM - 3:30 PM, Monday/Tuesday/Thursday/Friday 3:00 PM - 11:30 PM; Week 2 - Tuesday/Wednesday/Thursday 3:00 PM - 11:30 PM, Saturday 7:00 AM - 11:30 PM
Hours per Week: 40
Candidate Pay Rate: $33 per hour

Roles & Responsibilities
- Administer medications and treatments accurately and safely as ordered by physicians
- Monitor patient vitals, symptoms, and overall condition, reporting changes promptly to the nursing team
- Provide compassionate assistance with activities of daily living, including feeding, dressing, and mobility
- Document patient care thoroughly and timely in the electronic health record
- Collaborate with nurses, doctors, and other healthcare professionals to deliver comprehensive care
- Educate patients and their families on care plans and health management
Qualifications
- Active Licensed Practical Nurse (LPN) license in New Hampshire or a compact state
- Must have completed the original or updated COVID-19 vaccine or present a documented religious or medical exemption
- Ability to work evening shifts and alternating weekend schedules
- Prior LPN experience in long-term care or skilled nursing is a plus, but new graduates are encouraged to apply
- Strong clinical judgment and a compassionate, patient-centered approach

- Medication administration and dosage calculation
- Patient assessment and vital sign monitoring
- Accurate documentation and EHR charting
- Infection control and safety protocols
- Effective communication and teamwork
- Ability to adapt to a fast-paced, resident-focused environment
