Roles & Responsibilities

• Administer medications and treatments accurately and safely as ordered by physicians
• Monitor patient vitals, symptoms, and overall condition, reporting changes promptly to the nursing team
• Provide compassionate assistance with activities of daily living, including feeding, dressing, and mobility
• Document patient care thoroughly and timely in the electronic health record
• Collaborate with nurses, doctors, and other healthcare professionals to deliver comprehensive care
• Educate patients and their families on care plans and health management

Qualifications

• Active Licensed Practical Nurse (LPN) license in New Hampshire or a compact state
• Must have completed the original or updated COVID-19 vaccine or present a documented religious or medical exemption
• Ability to work evening shifts and alternating weekend schedules
• Prior LPN experience in long-term care or skilled nursing is a plus, but new graduates are encouraged to apply
• Strong clinical judgment and a compassionate, patient-centered approach
