Roles & Responsibilities

• Provide direct nursing care to residents with compassion and accuracy
• Administer medications and document per facility policy and scope of practice
• Perform routine assessments and report changes to the care team promptly
• Assist with ADLs (activities of daily living) while promoting resident independence
• Collaborate with RNs, CNAs, and interdisciplinary team members for high-quality care
• Maintain clear, timely charting in the patient record and support continuity of care
• Follow infection control and safety protocols to protect residents and staff

Qualifications

• Current LPN license in Pennsylvania (must be active and in good standing)
• CPR/BLS certification (or willingness to obtain upon hire)
• Valid clinical immunizations and ability to pass background/health screenings
• Comfortable working in a long-term care/rehabilitation setting and providing hands-on care
• Strong communication skills and a positive, team-oriented attitude

Education

• Practical Nursing Diploma, Vocational Nursing Certificate, or equivalent completed
• Completion of an accredited LPN program and state licensure

Knowledge & Skills

• Medication administration and accurate documentation
• Basic wound care and observation/reporting of skin integrity
• Resident assessment skills and recognizing changes in condition
• Familiarity with electronic health records or willingness to learn
• Time management, prioritization, and teamwork in a fast-paced setting
• Empathetic patient interaction and family communication
