Roles & Responsibilities
- Provide compassionate, direct patient care in accordance with care plans
- Administer medications and treatments safely, documenting accurately
- Monitor patient status and report changes to the clinical team promptly
- Support nursing assessments and assist with procedures as directed
- Communicate clearly with patients, families, and interdisciplinary staff
- Maintain accurate, timely charting and follow facility policies
- Contribute to a positive unit culture by mentoring peers and sharing best practices
Qualifications
- Current Licensed Practical Nurse (LPN) license in Pennsylvania
- Graduation from an accredited LPN program
- Basic Life Support (BLS) certification or willingness to obtain
- Clinical experience caring for adult or geriatric patients preferred
- Strong communication skills and a team-first mindset
- Ability to pass required background and health screenings
Education
- Graduate of an accredited LPN / Practical Nursing program
- High school diploma or GED required
Knowledge & Skills
- Solid medication administration and documentation skills
- Knowledge of basic nursing assessments and vital sign interpretation
- Comfort working in a fast-paced long-term/post-acute care environment
- Familiarity with electronic health records and standard charting practices
- Good time management and prioritization under shifting workloads
