Roles & Responsibilities
- Provide compassionate direct patient care and support daily living needs
- Administer medications and treatments safely and accurately
- Monitor, assess, and document patient vitals and condition changes
- Collaborate with nurses, physicians, and care team members to deliver coordinated care
- Maintain clear, timely patient records and communicate effectively with families
- Follow infection control, safety protocols, and facility policies to protect patients and staff
Qualifications
- Licensed Practical Nurse (LPN) — current Pennsylvania license or eligible to obtain licensure
- Basic Life Support (BLS) certification preferred or willing to obtain
- Comfortable working 8-hour clinical shifts and providing hands-on patient care
- Strong communication skills and the ability to work well within a care team
Education
- LPN diploma or vocational nursing certificate
Knowledge & Skills
- Patient assessment and monitoring
- Medication administration and documentation
- Basic wound care and ADL assistance
- Electronic health record documentation and charting
- Clear, compassionate communication with patients and families
- Teamwork and collaboration in a fast-paced healthcare environment
