Candidate Pay Rate: $34 per hour

Roles & Responsibilities

Provide compassionate direct patient care, including vital signs, ADLs, and bedside support
Administer medications and treatments safely according to policy and scope
Document patient care thoroughly and accurately in the chart/EHR
Communicate effectively with patients, families, and the interdisciplinary team
Assist with care planning and implement delegated nursing tasks
Monitor patients for changes in condition and escalate concerns promptly
Support a positive, safe environment that promotes dignity and recovery

Qualifications

Current Licensed Practical Nurse (LPN) license in Pennsylvania or compact/state-appropriate licensure
Minimum clinical experience caring for adults in a long-term care or similar setting preferred
Current Basic Life Support (BLS) / CPR certification or willingness to obtain
Up-to-date immunizations and ability to pass required background checks
Strong interpersonal skills and a patient-centered approach

Education

Graduate of an accredited Practical Nursing program; LPN diploma or equivalent required

Knowledge & Skills

Medication administration and documentation best practices
Skilled in taking vital signs and recognizing patient condition changes
Comfortable with electronic health record (EHR) documentation
Good clinical judgment, time management, and prioritization
Teamwork and clear communication with patients, families, and staff
