Hours per Week: 40

Candidate Pay Rate: $34 per hour

Roles & Responsibilities

Provide compassionate direct patient care, monitoring patients and responding to needs promptly
Administer medications and treatments safely and accurately per facility policy
Take and record vital signs, observe changes, and communicate findings to the care team
Document patient care thoroughly and maintain accurate medical records
Assist with activities of daily living and support patient comfort and dignity
Collaborate with RNs, therapists, and interdisciplinary staff to support individualized care plans
Participate in shift handoffs and effective communication to ensure continuity of care

Qualifications

Active Licensed Practical Nurse (LPN) license for Pennsylvania (required)
Current BLS/CPR certification (preferred or required per facility policy)
Ability to pass background checks and required health screenings
Strong communication skills and a patient-centered approach
Dependable, punctual, and able to work effectively as part of a care team

Education

Practical Nursing diploma, certificate, or equivalent LPN/Vocational Nursing education

Knowledge & Skills

Medication administration and basic clinical assessments
Accurate charting and familiarity with electronic medical records
Knowledge of infection control and safety practices
Comfortable assisting with ADLs and basic nursing procedures
Good critical thinking and the ability to escalate concerns appropriately
Teamwork, time management, and a compassionate bedside manner
