Roles & Responsibilities

Provide high-quality outpatient family medicine care for patients across the lifespan
Build and maintain long-term, patient-centered relationships emphasizing continuity of care
Diagnose, manage, and treat acute and chronic conditions with evidence-based practice
Collaborate closely with a multidisciplinary clinical and administrative team to coordinate patient care
Participate in community-focused initiatives to improve local health outcomes
Maintain accurate medical records, complete required documentation, and ensure regulatory compliance
Engage in quality improvement activities and professional development to advance clinical practice

Qualifications

MD or DO degree from an accredited medical school
Board Certified or Board Eligible in Family Medicine
Eligible for medical licensure in New York State
DEA certification or eligibility
Demonstrated commitment to community-focused, patient-centered primary care

Education

MD or DO from an accredited medical school

Knowledge & Skills

Strong clinical skills in family medicine and primary care
Experience with outpatient care and continuity of care models
Patient-centered approach with excellent communication and interpersonal skills
Ability to work effectively within a collaborative care team
Comfort practicing in a rural/community-focused setting
