Roles & Responsibilities

• Provide compassionate, direct patient care under the supervision of RNs and physicians
• Administer medications and treatments as ordered, documenting patient responses accurately
• Monitor and record vital signs, promptly reporting any changes to the care team
• Assist patients with daily living activities while promoting independence and dignity
• Collaborate with the interdisciplinary team to implement and update individualized care plans
• Educate patients and families on health conditions, medications, and post-discharge care

Qualifications

• Active Licensed Practical Nurse (LPN) license in the state of Pennsylvania
• Current BLS/CPR certification, or ability to obtain before starting
• Strong clinical assessment skills and attention to detail
• Ability to work effectively both independently and as part of a team
• New graduates welcome; previous LPN experience is a plus

Knowledge & Skills

• Medication administration and accurate dosage calculations
• Patient assessment and vital sign monitoring
• Electronic health record (EHR) documentation
• Empathy, patience, and excellent interpersonal skills
• Ability to prioritize tasks and adapt in a fast-paced environment
• Solid understanding of infection control and safety protocols
