Roles & Responsibilities

• Provide compassionate direct patient care and assist patients with activities of daily living
• Support safe transfers, ambulation and mobility assistance with attention to dignity
• Monitor, record and report vital signs and changes in patient condition
• Ensure clear, timely documentation in the patient record
• Collaborate with nurses and rehab staff to deliver coordinated care plans
• Maintain infection control standards and a clean, safe environment
• Communicate effectively with patients and families to support comfort and understanding

Qualifications

• Current CNA (Certified Nursing Assistant) certification — required
• Basic Life Support (BLS/CPR) certification preferred or required per facility policy
• Ability to pass background check and drug screen as part of onboarding
• Reliable local transportation — this is a local-only assignment
• Strong communication skills and a patient-centered attitude
• Previous experience in long-term care or rehabilitation settings is a plus

Education

• Completion of an approved CNA training program; high school diploma or equivalent preferred

Knowledge & Skills

• Comfort with patient hygiene, ADLs and mobility support
• Accurate vital sign collection and basic clinical observations
• Familiarity with documentation and basic electronic medical records
• Understanding of infection control and safety procedures
• Time management and teamwork in a fast-paced care environment
• Compassionate bedside manner and ability to follow care plans
