Location: Prescott, Arizona

Contract Type: Contract

Shift Type: Monday-Friday, 8:00 AM - 4:30 PM, occasional weekends every 3-4 weeks, flexibility for ED coverage 10:00 AM - 6:30 PM

Hours per Week: 40

Candidate Pay Rate: $50 per hour

Roles & Responsibilities

Conduct comprehensive social work assessments to support patient needs
Perform mental health assessments and Title 36 petitions as required
Develop discharge plans that pave the way for smooth transitions
Address homelessness, substance abuse, and SDOH through targeted consults
Complete maternal child assessments and handle DCS/APS reporting
Serve diverse areas including inpatient, ED, Family Birthing Center, and ICU

Qualifications

Active LMSW or LCSW license
At least 1 year of experience
Available for profile review and manager interview
