Candidate Pay Rate: $49 per hour

Roles & Responsibilities
- Deliver compassionate care to patients across a variety of diagnoses including cardio, pulmonary, orthopedic, neurological, and geriatric conditions
- Provide personalized physical therapy treatments in patients' homes throughout the greater Sacramento county area
- Collaborate with interdisciplinary teams to create and implement effective care plans
- Track and document patient progress while meeting productivity goals of 5-6 patients per day
- Build meaningful relationships with patients and their families while supporting their recovery journey
Qualifications
- Active Physical Therapy Assistant license in California
- Minimum experience requirements vary, new grads considered in extreme cases only
- Strong communication and interpersonal skills
- Ability to work independently in home health settings
- Valid driver's license and reliable transportation
Knowledge & Skills
- Home health care experience preferred
- Knowledge of various diagnoses including cardio, pulmonary, orthopedic, neurological, and geriatric conditions
- Strong documentation and time management abilities
- Comfortable working independently in diverse home environments
