Roles & Responsibilities
- Provide compassionate physical therapy services to patients in their homes according to physician care plans
- Assess and evaluate patients' therapeutic needs, muscle strength, mobility, gait, and rehabilitation potential
- Develop and implement personalized treatment plans that help patients regain independence
- Educate patients and families on therapeutic exercises, equipment use, and home safety
- Collaborate with physicians and care teams to report progress and adjust treatment goals
- Supervise home health aides and coordinate with other healthcare professionals for comprehensive care
- Document clinical progress and maintain accurate patient records to support quality outcomes
Qualifications
- Minimum 2 years experience as a Physical Therapist in acute, home care, or rehab settings
- Current BLS certification required
- Strong oral and written communication skills with excellent organization abilities
- Valid Physical Therapy license for California practice
- Ability to work independently while providing exceptional patient-centered care
