Roles & Responsibilities
- Deliver personalized physical therapy services to patients recovering from surgeries, falls, and other conditions in their homes
- Develop and implement individualized treatment plans that help patients regain mobility and independence
- Collaborate with home health and hospice teams to ensure seamless, patient-centered care
- Document patient progress accurately using the HomeCare/HomeBase EMR system
- Travel within a 60-mile radius around Valley City to provide care where patients need it most
Qualifications
- Current North Dakota Physical Therapy License
- BLS certification required
- At least 1 year of physical therapy experience, including previous home health experience
- Valid driver's license and auto insurance
- Comfortable working independently without on-site manager supervision
Knowledge & Skills
- Strong clinical skills in physical therapy and home health settings
- Ability to manage a caseload of 5-7 patients daily
- Familiarity with HomeCare/HomeBase EMR preferred
- Excellent time management and organizational abilities
- Compassionate approach to caring for home health and hospice patients
