Roles & Responsibilities

• Conduct comprehensive patient assessments to evaluate physical function and mobility.
• Develop and implement individualized treatment plans to restore function, reduce pain, and improve quality of life.
• Collaborate with a multi-disciplinary team to coordinate patient care and achieve optimal outcomes.
• Educate patients and families on therapeutic exercises and activities for home management.
• Maintain accurate and timely documentation of patient progress and treatment outcomes.

Qualifications

• Valid Physical Therapist license in Pennsylvania
• Doctor of Physical Therapy degree from an accredited program
• Experience in geriatric or skilled nursing settings is a plus
• Excellent communication and interpersonal skills
• Commitment to providing high-quality, compassionate care

Knowledge & Skills

• Strong clinical reasoning and assessment skills
• Proficiency in developing and executing treatment plans
• Knowledge of evidence-based physical therapy practices
• Ability to work effectively in a team environment
• Excellent documentation skills
