Roles & Responsibilities

• Evaluate and assess patients' physical therapy needs in a skilled nursing setting
• Develop and implement individualized treatment plans to improve mobility and function
• Collaborate with interdisciplinary teams to ensure comprehensive patient care
• Document patient progress and adjust treatments as needed
• Educate patients and families on therapeutic exercises and home care
• Comply with all regulatory standards and facility policies

Qualifications

• Valid Physical Therapist license in North Carolina or eligibility for licensure
• Graduate from an accredited Physical Therapy program
• Experience in skilled nursing or long-term care preferred
• Excellent communication and interpersonal skills
• Ability to work effectively in a team-oriented environment

Knowledge & Skills

• Knowledge of physical therapy techniques and modalities
• Familiarity with Medicare and rehab documentation
• Strong critical thinking and problem-solving abilities
• Compassionate and patient-centered approach
