Roles & Responsibilities

• Evaluate and assess patients' physical therapy needs in a skilled nursing facility
• Develop and implement individualized treatment plans to improve mobility and function
• Provide therapeutic exercises and modalities to help patients reach their goals
• Collaborate with interdisciplinary team members to ensure comprehensive patient care
• Document patient progress and adjust treatments as needed
• Educate patients and families on therapeutic techniques and home exercises

Qualifications

• Current Physical Therapist (PT) license in the state of Oklahoma
• Previous experience in skilled nursing or rehab settings preferred
• Strong clinical assessment and treatment planning skills
• Excellent communication and interpersonal abilities
• Ability to work effectively as part of a multidisciplinary team

Knowledge & Skills

• Knowledge of physical therapy modalities, therapeutic exercises, and patient care techniques
• Understanding of Medicare/Medicaid regulations and documentation requirements
• Proficiency in electronic health records (EHR) systems
• Strong organizational and time-management skills
• Compassionate and patient-centered approach
