Roles & Responsibilities

  • Conduct comprehensive physical therapy evaluations for senior residents, focusing on functional mobility and independence.
  • Develop and implement individualized treatment plans that help residents achieve their goals and improve quality of life.
  • Collaborate with the interdisciplinary care team, including nurses, physicians, and occupational therapists, to ensure holistic care.
  • Document patient progress accurately, adjusting plans as needed to optimize outcomes.
  • Educate residents and their families on safe movement, fall prevention, and home exercise programs.

Qualifications

  • Active Physical Therapist license in Pennsylvania (or eligibility to obtain).
  • Graduate of an accredited Physical Therapy program (DPT preferred).
  • Previous experience in senior care, skilled nursing, or rehabilitation settings is a plus.
  • Strong communication and interpersonal skills to work effectively with older adults and their families.
  • Must pass background checks and comply with facility policies.

Knowledge & Skills

  • In-depth knowledge of geriatric assessment and treatment techniques.
  • Ability to develop and modify treatment plans for a variety of conditions, including post-surgical, orthopedic, and neurological issues.
  • Excellent documentation skills with attention to detail.
  • Familiarity with EHR systems and therapy software.
